All potential clients must sign a release of liability/health form prior to service being requested. Full payment requested for all services 24hrs in advance to appointment date. Debit, Credit, Cash and Check accepted. Debit and Credit through PayPal only. Service will not be performed until exchange of monies has cleared. All checks require proof of valid drivers license. All checks are cashed same day received. NSF/Cancelled checks will be reported and processed as well as FULL SERVICE FEE plus $25.00 NSF/Cancellation fee being charged for time slot provided for potential service requested. If failure to pay, you will be reported to the credit bureau as well as collections. Please notify your therapist within 24hrs if you need to cancel your appointment time. You may only reschedule your missed/cancelled appointment. There are NO REFUNDS in exchange for timeslot provided to potential service request. Thank you for your respect!
